The Affiliate Application


Download and complete in full detail each of the appropriate applications from the list below. Do not return partially filled out forms to us as this will greatly slow down the application process


Please submit application between 8:00 AM and 3:00 PM, Monday - Friday



Step 1:

  1. The PPAR Affiliate Membership Application (required for all Affiliates) Fees are listed on the Affiliate application. Please call Angela Blazo at 719-633-7718 Option #3 for questions on Affiliate Membership.
  2. The Marketing Communication Consent Form (Form required for all affiliates).

Step 2:

After all forms are completed, return the forms along with application fees and dues to PPAR:
  • Bring it to our offices located at 430 N. Tejon Street, Suite #101 BEFORE 3:00 Monday-Friday
  • Email a PDF of the packet to ablazo@ppar.org
